- Please note: The self-service site is available 24/7. You can modify your benefit elections only during Open Enrollment or if you experience a Life Event. However, you may update beneficiaries, download your benefit confirmation statement, and process Life Events throughout the year.

Benefits Enrollment Information for Newly Eligible Employees
Qualifying Life Event (QLE): HR must be notified within 30 days of the Qualifying Event, and proof is required; email supporting documentation to [email protected].
Domestic Partners are not eligible – New Dependent documentation must be submitted to HR at [email protected] after completing your enrollment elections: Marriage Certificate or Divorce Decree, Court orders requiring coverage, Birth certificate for the child(ren), and Social Security cards for all new dependents.
• Flexible Spending Account (FSA) and Dependent Care FSA DO NOT roll over and must be re-elected and submitted annually.
• Name changes must be sent to HR at [email protected].
• You are eligible for the medical waiver if you opt out of medical. A copy of your insurance card must be submitted to HR at [email protected] before the end of the month after completing your enrollment process.

The Madison National Life Insurance Company requires Evidence of Insurability (EOI) for the following benefits:

- Voluntary Group Life: a newly hired employee who applies for coverage over $50K or 1 times salary, whichever is less, will be required to complete an EOI. It must be submitted directly to the carrier within 60 days of your enrollment process.
- Long-Term Disability: Only new hires who enroll in the first 31 days of employment will be eligible to enroll without completing an EOI.
